Project Description:
Syphon Interactive was selected to create a survey management
system to allow the Johns Hopkins Arthritis group to collect demographic information,
suggestions, symptoms, and other helpful information from their web site visitors.
The Survey Manager we developed uses a wizard that guides an administrator step-by-step
through the process of creating a survey. Surveys can be stored indefinitely, launched at any time
and any number of times, and can be set to automatically run between specific dates. Development also included the interface that site visitors use to take the surveys and a means of retrieving
and downloading the collected data for analysis.
Some of the features include:
- Wizard guides administrator through developing short-answer and multiple choice questions using every means of user input allowable in an HTML form, with no knowledge of HTML required.
- Surveys can include optional customizable titles, headers, and footers.
- Surveys can list all questions at once or in chunks of any number of questions per page.
- Questions can be linked together to form single multi-part questions.
- Survey run time can be controlled manually or to automatically run between specific dates.
- Surveys can be set to launch as a pop-up window or to be availaible as a link on a list of available surveys.
- Users can only take a survey once and can choose to skip a survey and not be asked about it again.
- Users taking a multi-page survey can partially complete the survey and return to finish at a later time.
- Collected data downloads in Microsoft Excel format.
